*Duties and responsibilities*
Responsible for the following duties listed and any other tasks asked within reasonable expectation by supervisor
* Collaborates with all staff to determine continuum of care for program participants
* Directs program RA staff in the day-to-day operations of the transitional program
* Ensures that RA staff have been adequately trained in the day-to-day operations of the transitional program
* Assesses and evaluates staff; develops methods to use and enhance strengths and to isolate and minimize weaknesses
* Acts as the program’s liaison with families
* Attends meetings called by the executive staff/supervisor
* Participates in quality assurance, quality improvement and utilization review activities and makes recommendations to the supervisor for plans of corrections
* Maintains established performance and conduct standards in accordance with organizational expectations
* Observe all rules of confidentiality relating to clinical information and treatment, both internally and when dealing with external agencies and/or individuals
* Responsible for understanding clients rights policy and procedures
* Participate in staff training and development
* Conducting annual evaluations for all responsible staff and staff development plans
* Maintain professional standards at all times and observe the guidelines established within code of ethics
Qualifications:
CDCA Preferred
High School Diploma or GED
Valid Driver’s License
Successful completion of pre-employment drug screen and TB test
Successful completion of Local and BCI Background Check
