The Case Manager facilitates and monitors the provision of cost-effective healthcare services that meets the patient’s needs in a timely manner, in the most clinically appropriate setting.
Collaborates with the patient/family, physician, healthcare team, payer representatives, utilization review and post-acute service providers to assess, plan, implement, coordinate, monitor and evaluate options and services to promote optimal patient outcomes.
Ensures compliance with quality patient care and regulatory compliance.
The Case Manager provides care management, care coordination and discharge planning to ensure that the patient progresses through the continuum of care.
Serves as a liaison for patients/families in the healthcare system and the community to coordinate self-management support and appropriate post-acute referrals of chronic diseases for assigned populations.
Utilizes team approach to promote and maintain plan of care, problem solving skills and nursing process to achieve optimal patient outcomes.
Has responsibility to direct shift activities for Occupational Therapy consistent with departmental and corporate policies and procedures including flexing staff based on operational needs and enter time in time keeping system for designated employees.
May provide input only for evaluations, hiring, coaching, and skills development, and discipline.

EDUCATION
Bachelor's Degree with concentration in Physical Therapy, Occupational Therapy, or Speech Therapy
REQUIRED CERTIFICATION
Current Ohio License as a Physical, Occupational, or Speech Therapist
