*Job purpose* Provides medical services to clients in residential, transitional, or out-patient settings to ensure individualized client needs are met according to agency policy *Duties and responsibilities* Responsible for the following duties listed and any other tasks asked within reasonable expectation by supervisor * Ensure accurate and proficient client documentation * Furthers the mission of the organization through active support of the strategic goals * Develop and maintain professional working relationships with agency staff, clients, and outside entities * Provide medical education on appropriate topic to clients * Effectively de-escalate crisis situations * Conduct nursing assessments of assigned clients * Evaluating & determining medical stability for level of care * Identify & implementing continuity of care * Facilitate nurse groups as needed * Establishing and supporting routine exercise program, healthy eating, and sober and healthy lifestyles * Identify opportunities for health promotion and illness prevention * Coordinate with other departments to ensure all the client’s needs are met * Review records to ensure medical quality assurance and completeness * Provide medical staff training as needed * Ensure special medical needs of clients are identified & addressed * Complete client medical documentation within timely manner * Send referrals as needed * Advocate for clients * Provide billable services that are applicable to licensures * Observe all rules of confidentiality relating to clinical information and treatment, both internally and when dealing with external agencies and/or individuals * Responsible for understanding clients rights policy and procedures * Participate in staff training and development * Maintain professional standards at all times and observe the guidelines established within code of ethics and agency policy * Licensed Practical Nurse license required * Professional knowledge of comprehensive clinical assessment skills * Familiar with community recovery/social support resources * Ability to follow oral and written instructions * Organizational skills Job Type: Full-time Benefits: * 401(k) * 401(k) matching * Dental insurance * Flexible spending account * Health insurance * Life insurance * Paid time off * Vision insurance Medical specialties: * Addiction Medicine Standard shift: * Day shift Weekly schedule: * 4x10 * 5x8 People with a criminal record are encouraged to apply Work Location: In person
